The FDA in the United States, for

That might not sound like a huge amount,

but it's actually a pretty meaningful

clinical threshold. It's the kind of

disease. I mention this because as we go

Right, let's get into it. First up is

something called bitter orange, also

known as citrus aurantium.

Or if you've ever eaten a proper British

marmalade, worst type of jam ever by the

way, those are Seville oranges, [music]

which are the same thing. The extract is

taken from the peel and its main active

ingredient is a compound called

synephrine. So, synephrine is a

stimulant and if that rings alarm bells,

it should. Bitter orange essentially

stepped into the spotlight after another

stimulant called ephedra was banned in

the early 2000s

>> following serious safety concerns,

including reports of cardiac events and

even some deaths. Synephrine has a

similar mechanism of action to

ephedrine, stimulating the nervous

system in a way that is supposed to

breakdown. So, does it work? Well,

here's the thing. The review I mentioned

found that between 2006 and 2016,

Not one.

So, in terms of efficacy, we genuinely

have nothing to go on. What we do have

is a case study of a patient who took

bitter orange and ended up in the

emergency room with kidney [music]

and respiratory failure. Now, a single

case study is not enough to condemn a

supplement outright and a separate

safety trial in 75 people over 60 days

reported no adverse events at all. But,

given that bitter orange has essentially

replaced a banned substance, has a

similar mechanism of action and has zero

weight loss evidence to its name, the

risk-reward calculation there

isn't a great one. Now, next is

something more of you will have heard of

before.

Carnitine, specifically L-carnitine.

This one actually has a pretty

compelling mechanism

on paper. Carnitine is a compound that

your body produces naturally and it's
4:08
found in foods, too, particularly in red

meat. Its job in the body is to
4:12
transport fatty acids into the
4:14
mitochondria, the energy-producing
4:16
engines of your cells. So, the logic

goes like this: [music] the more

carnitine, the more fat transported, the

more fat burned.

Sounds reasonable, right? The problem is

that having a logical mechanism [music]

and actually producing meaningful weight

loss in the real world are two very

different things. The review I mentioned

found just one study on carnitine that

met their criteria and it showed no

significant difference in weight loss

compared to placebo.

Pretty damning for a supplement with

such a long [music] history. However,

since that review was published, the
4:49
picture has become slightly more
4:52
nuanced. A more recent meta-analysis,

pooling data from dozens [music] of

randomized controlled trials, have found
4:57
that L-carnitine supplement does produce

a small but statistically significant
5:03
reduction in body weight. The reduction

was around 1 kg compared to placebo,

with the effect being most [music]

consistent in people who are overweight

or obese

and when combined with other lifestyle

changes, which is very much worth

bearing in mind. But, here's something

else worth bearing in mind.

variability between studies,

percentage in many of the studies. So,

signal there, but whether that

translates into something actually

meaningful for most people is a very

It's also worth pointing out that if you

meat regularly, your carnitine levels

are probably already okay.

Your body makes its own carnitine, too.

So, the idea that supplementing extra

will push fat burning into some higher

gear isn't really well supported by the

overall [music] evidence. Moving on to

one that most people have probably never

heard of. Coleus forskohlii is a plant

from the mint family native to India.

And it's been used in traditional
6:11
Ayurvedic medicine for centuries. The

root of the plant contains a compound

called forskolin, which has attracted
6:18
interest for a range of health effects,
6:21
including weight loss. The proposed
6:23
mechanism involves forskolin activating
6:25
an enzyme called adenylate cyclase,
6:28
which increases levels of a molecule
6:30
called cyclic AMP in cells. Cyclic AMP
6:34
is often elevated in an energy deficit,
6:36
so can tell other systems that the body
6:39
needs energy. The idea is that if cyclic

AMP is higher, it might promote the

breakdown of stored fat and increase

lean body mass. As mechanisms go, it's

actually fairly well established at a

cellular level, and some animal studies

have shown interesting effects on fat

cell size and glucose metabolism. The

human evidence, however, is a lot less

impressive. The overall body of clinical

The best available studies, including

the one examined in the review, are

but the evidence base remains way too

limited and inconsistent to draw firm

conclusions. At this point, there are

just not enough high-quality human

prophylactically. There were also some

adverse events reported across studies.

Some participants had mild

gastrointestinal symptoms, specifically

loose stools, which resolved within a

few weeks, but doesn't sound [music] all

that fun to be honest. Next up, chromium

is an essential trace mineral that your

body needs in small amounts. You get it

from food, particularly whole grains,

[music] meat, and broccoli. Uh and it's

been studied since the 1970s for its

role in helping insulin work properly in

the body.

And it's that insulin connection that

led to its popularity as a weight loss

supplement. The proposed mechanism is

like this.

Because chromium supports insulin's
8:21
action, and insulin plays a central role
8:24
in regulating fat storage and
8:25
metabolism,

the thinking was that supplementing with

chromium, specifically in a highly
8:30
absorbable form called chromium

picolinate, might improve the body's
8:35
ability to burn fat and reduce appetite.

It sounds possible, right? It's also the
8:40
kind of logic that can sound a lot more
8:42
convincing in a marketing brochure than
8:44
it does [music] in a clinical trial. The
8:46
review I mentioned looked at seven
8:48
studies on chromium picolinate, [music]

and the majority found no statistically
8:52
significant effect on body weight.

One meta-analysis, pooling data from 10

randomized controlled trials, did find a

small, significant reduction in body

placebo. But, the authors themselves

stated that the result was heavily

And the clinical relevance of that

at all. Where the picture gets slightly

more interesting is with insulin

There is some evidence that chromium

are insulin resistant, potentially

improving blood glucose regulation. But,

that's a metabolic health finding, not

fat loss finding. And it's most relevant

to a very specific clinical population,

rather than the general gym-going

public. In terms of safety, chromium

picolinate is generally well tolerated

at the doses used in supplements. A

handful of participants across different

trials did withdraw due to side effects

including nausea and vertigo, but these

weren't common. The picture here is

fairly clear.

Chromium picolinate might do something

very small for body weight in certain

individuals, but there is not enough

evidence to recommend it as a meaningful

fat loss supplement. Now, a lot of you

have probably heard of conjugated

linoleic acid [music] or CLA. It's a

type of fatty acid that's found

naturally in meat and dairy products

from ruminant animals like cows and

sheep.

So, [snorts] if you eat beef, lamb, or

full-fat dairy, you're probably getting

some CLA in your diet. It became popular

as a supplement largely on the back of
10:36
some impressive results in animal
10:38
studies, where it considerably reduced
10:40
body fat. The human data, however,
10:41
[music] has been a different story. The

proposed mechanisms are actually pretty
10:45
interesting. CLA is thought to influence
10:48
fat metabolism by promoting the
10:50
breakdown of stored fat, reducing fat
10:53
cell formation, and potentially
10:55
increasing the amount of energy burned

at rest. There are two main forms of CLA

used in supplements, and they appear to

work through different pathways, which

added a layer of complexity to

interpreting the research. [music] The

review I mentioned examined 11 studies

on CLA in people with obesity, and the

majority found no significant difference

in weight [music] loss between the CLA

and the placebo groups. Where weight

loss did appear greater in CLA [music]

groups, it tended to occur in studies

calorie restriction or exercise

And that makes it very difficult to

separate the effect of the supplement

from the effect of the lifestyle change.

To be honest, that's a problem in a lot

of supplement research. More recent

meta-analyses paint a slightly more

complicated picture.

If you pull data from multiple

analyses do find small but statistically

fat mass, and body fat percentage

That's less than the weight of a pound.

And more importantly, when researchers

That's a red flag worth paying attention

to. There is one mildly interesting

finding worth mentioning.

A more recent meta-analysis found that

CLA combined with exercise may produce

to exercise alone without a significant

effect on body weight. That could

suggest a modest body composition

On the safety side, most adverse events

This is worth keeping in mind since many

to be practically meaningful for most

delivered.

Right, this is one of the more

interesting ones. Partly because you've

almost [music] certainly eaten the

source of this supplement without

knowing. Capsinoids are compounds found

in chili peppers. Capsaicin is the one
13:29
most people have heard of. It's what
13:30
gives chili peppers [music] their heat.

Capsinoid supplements typically contain
13:35
capsaicin or another non-spicy capsinoid
13:38
or a combination of both. The mechanism
13:40
for how they work is actually pretty
13:42
well understood at a physiological
13:43
level.
13:44
Capsaicin binds to receptors which are
13:46
found in sensory nerve cells throughout
13:48
the body, including in the gut.

Activating these receptors can stimulate
13:52
thermogenesis, meaning the body produces
13:54
more heat, which burns more energy.

There's also some evidence that

capsaicin and capsinoids can activate

brown adipose tissue, the type of fat

that actually burns energy rather than
14:04
just storing it. On top of that, it may

have a mild appetite-suppressing effect

as well.

On paper, this is actually one of the

more interesting fat loss mechanisms of

any supplement we've looked at so far.

So, what does the evidence actually

show?

The review I mentioned examined two

randomized controlled trials on

capsinoids specifically. In both of

them, participants lost a similar amount

of weight to those in the placebo group.

In one study, both groups [music] lost

around 0.5 to 0.9 kg over 12 weeks with

no significant difference between them.

In the other study, both groups lost

weight. Next up is psyllium, which is

actually one [music] of my favorite

supplements, but for its health

benefits.

It's a type of soluble dietary fiber

that we get from the husks of seeds from
16:17
a plant called plantago [music] ovata.

You've actually probably seen it sold
16:21
under brand names like Metamucil, and
16:22
it's been used for decades as a
16:24
treatment for constipation.

Already sounds super sexy, right?
16:29
Well, more recently, [music] it's been
16:31
marketed as a weight loss supplement,
16:32
too. So, let's see if its reputation is
16:35
deserved. [music]

The mechanism for this one is pretty
16:38
straightforward.
16:39
When psyllium comes into contact with
16:41
water, it forms a thick, viscous gel in
16:44
the gut. That gel slows down the rate at
16:46
which food leaves your stomach, which
16:48
promotes a feeling of fullness and
16:50
reduces how quickly nutrients, including
16:52
glucose, are absorbed into the
16:54
bloodstream. The idea is that if you
16:56
feel fuller for longer, you eat less,
16:59
and over time, that adds up to weight

loss. So, it's a satiety-based mechanism
17:04
rather than a thermogenic one, which
17:06
makes it quite different from a lot of
17:08
the other supplements we've talked

about. The review I mentioned looked at

three studies on psyllium. Two of them,

So, that suggests that timing might

actually much more consistent and robust

cholesterol and improves blood glucose

regulation. So much so that it's an

approved ingredient for

cholesterol-related health claims in

several countries. So, psyllium might

still be worth considering for some

people who are more concerned with

metabolic health. The most common side

effects were things like bloating and

loose stools, which again is pretty

normal for any significant increase in
18:33
dietary fiber intake. A big practical
18:36
note [music] is that psyllium has to be

taken with enough water because
18:40
otherwise, it'll try to absorb the water

from your digestive system, which
18:44
[music] you don't want happening. Right,

here's another one you probably never
18:48
heard of. But, if you've ever eaten

konjac noodles or seen those slightly
18:52
translucent rubbery noodles in Asian
18:54
grocery stores that seem to have almost
18:56
no calories, then you already know about
18:59
glucomannan. It's another soluble
19:02
dietary fiber extracted from the root of
19:04
the konjac plant, which has been [music]
19:05
used in Asian cooking and traditional
19:07
medicine for thousands of years. In
19:09
Japan and China, it turns up in all
19:11
sorts of foods, including [music]
19:12
noodles, tofu, and jellies. As a
19:14
supplement, it's been very heavily
19:16
marketed for weight loss, and this one
19:19
might actually have some of the more
19:20
interesting evidence of anything we've
19:23
looked at in this video. Again, the
19:24
mechanism is a lot like psyllium.

Glucomannan is [music] really viscous,
19:29
meaning when it absorbs water, it forms
19:31
a very thick gel in the stomach. In
19:33
fact, it's one of the most viscous
19:34
dietary fibers known. [music] That gel
19:36
slows down gastric emptying, keeps you
19:39
feeling full, and blunts the rise in
19:40
blood glucose after a meal. So again,
19:42
it's mostly a satiety-based [music]
19:44
mechanism. But the viscosity of
19:46
glucomannan is a lot higher than most
19:49
other fibers, which is why some
19:50
researchers think it might be more
19:52
effective than something like psyllium

for weight management. The review I

mentioned found six studies on

glucomannan, [music] and the results

were kind of all over the place.

Some found small but significant

reductions in weight, but others found

no significant difference compared to

the placebo at all. One important

pattern that emerged was that a

specific, higher viscosity form of

glucomannan called PGX seemed to do a

to matter quite a bit here. The broader

lives up to the hype. You already know

green tea. It's one of the most consumed

drinks in the world, made from the

leaves of the Camellia [music] sinensis

plant, the exact same plant that makes

you black tea and oolong tea.

The difference is that green tea leaves

aren't fermented, which preserves their

polyphenol content. Green tea extract is

essentially a concentrated form of those

polyphenols, particularly a compound

active ingredient people are after. The

mechanism here is different from the

fiber-based supplements we just talked
