The incident occurred at the RK Jain Bharat Petroleum pump near Howrah Bridge, within the Golmuri police station limits of Jamshedpur, Jharkhand. A pipe suddenly burst while a vehicle was being refueled with CNG. A loud explosion was followed by a gas leak, triggering panic at the scene; people abandoned their vehicles and scrambled to flee. The video clearly shows the high-pressure CNG pipe suddenly detaching during the refilling process. Its heavy nozzle struck 24-year-old employee Tushar Khamri on the head and face with such force that he did not even have a split second to react or protect himself. He succumbed to his injuries while undergoing treatment at the hospital.
